Our online sports registration system is now live! To access it, go to www.phnathletics.com and click on FinalForms. FinalForms needs parents to create an account and it will ask them for their birthday. Once an account is created, an email will be sent to the parents and from there it will link them to our online registration forms. This process may take about 20- 30 minutes from beginning to end. Since medical forms have been turned in, parents can bypass these forms by hitting the skip feature at the bottom of the form. Finally, once parents have completed their portion, the students will have to create an account using their student email address. The students portion will take about 5-10 minutes to complete. Payment can be made with cash, check or credit card. If you send cash or checks, please make sure you have the students name & amount on an envelope or paperclipped to the money. Students can always drop by the athletic office to pay for their sports fees. Computers and internet access will be available upon request at PHN if anyone should need it. If paying by credit card, I would highly suggest paying through the phasd.us website (cheaper transaction fee). Parents will need their student's ID number (not a phone number) when prompted. If you have any questions, I can help parents through this process. If you would like to use eventlink to pay, it is higher transaction fee.
